Most rejected public service applications never reach a panel. They are screened out on administrative grounds before anyone assesses the candidate.
Screening happens first
Before shortlisting, HR checks each application against a compliance list. Anything failing that check is removed regardless of merit. It is not a judgement on your ability — it is a process step most applicants do not know exists.
The common reasons

The subtler ones
Not addressing the stated requirements
Your CV may show you can do the job without showing it against the criteria the panel scores. If the advert lists five requirements, a reader should be able to find evidence for each without hunting.
Generic applications
The same CV sent to thirty posts will underperform three tailored ones. Panels notice when nothing in a CV relates to their advert.
Formatting that fails on paper
Applications get printed. Columns, graphics and colour blocks reproduce badly and can render text unreadable.
If you are never shortlisted
Assume it is administrative until you have ruled that out. Check your Z83 completeness, your qualification levels against the adverts, and whether your CV is written for public sector scoring.
